民國初期，北方偏遠小鎮古樓鎮自二十年前開始受神祕怪病‘骷髏症’困擾，患病者言行怪異常受幻覺困擾，並且厭食嗜水，病人個個瘦骨嶙峋精神不振，因此古樓鎮又被周邊百姓戲稱為‘骨頭鎮’。近年來骨頭鎮周邊又添外患，從五年前開始，附近榛子林裡出現了一隻被稱為‘黑瞎子’的神祕野獸，時常跑進鎮裡滋擾百姓，令骨頭鎮更是雪上加霜。骨頭鎮裡，少女三更自幼跟隨師父孫老海學習獵術，練就一身例不虛發的精絕箭術，不料五年前黑瞎子突然出現，孫老海在獵殺黑瞎子途中遇害身亡。以至於少女三更暗暗起誓，終身以獵殺黑瞎子為己任，因此常徘徊在附近榛子林內尋找黑瞎子蹤跡。一夜，遊方小郎中白小白路過骨頭鎮外榛子林，突遭黑瞎子襲擊，多虧三更及時出現救下白小白一命，三更驚奇發現，黑瞎子不知為何竟恐懼白小白隨身藥箱中的藥品，但詢問白小白後，白小白自己也不清楚原因，為查明真相，三更蠻橫的降白小白強扣回骨頭鎮試圖破解謎團。來到鎮上的白小白發現許多鎮民身患骷髏怪病，通過醫生的敏銳嗅覺，發現一切並非表面看著的這麼簡單，但是對於他的危機也悄悄降臨。。。
